Community Health Worker (SDOH)
Position: Full-time Community Health Worker (SDOH)
Pay Rate: $22.00 per hour
Schedule: Monday–Friday, 40-hour work week
Summary: The Community Health Worker (CHW) (SDOH) is responsible for improving outcomes for high-risk patients, who frequently utilize the emergency department (ED) and/or hospital and have been identified through our Social Determinants of Health (SDOH) screening tool, PRAPARE. This position will help achieve better outcomes by working with an interdisciplinary team. The CHW provides services during clinic hours, participates in team huddles, and assists with patient intakes, education, and counseling in accordance with clinic protocols.
Qualifications and Abilities:
- Familiarity with common software programs, (i.e. Windows-based programs) and the ability to quickly learn new systems
- Ability to communicate effectively with patients, community members, and staff
- Proficiency in basic computer skills, including typing, navigating the internet and emails
- OSHA training
- BCLS certification (renewed biannually)
General Duties (include, but are not limited to):
- Serve as a patient advocate and facilitator of the Patient-Centered Medical Home (PCMH) and its target population of high-risk SDOH patients and high ED/hospital utilizers by providing health education, follow-up monitoring, and improving access to care
- Assess patients’ social needs by completing and reviewing the SDOH PRAPARE assessment form
- Identify SDOH-related needs and provide appropriate referrals to social services and community resources
- Develop specific, measurable, achievable, relevant, and time-bound (SMART) goals with patients
- Develop and implement a care plan tailored to the needs of the patient and their immediate support system. Through a patient-centered approach, the goal of this relationship with the patient is to decrease health disparities and improve health outcomes and addressing the patient’s SDOH
- Other duties as assigned
