RN Care Coordinator

Position: Full-Time RN Care Coordinator
Pay Rate: $79,160 annually
Schedule: Monday–Friday, 40-hour work week

Job Summary: The RN Care Coordinator works within a multi-disciplinary health care team to deliver comprehensive services for patients identified as high utilizers of the emergency department (ED) or hospital and/or diagnosed with chronic diseases, including but not limited to diabetes and hypertension. The RN Care Coordinator serves as a patient advocate and facilitator of the Patient-Centered Medical Home (PCMH) and its target population by providing health education, medication adherence counseling, follow-up monitoring, improving access to care, and guiding patients through problem-focused clinical visits.

Qualifications and abilities:

  • Current, valid Registered Nurse license in the State of Hawaii in good standing
  • Familiarity with common software programs (Windows-based systems) and ability to learn new systems quickly
  • Ability to communicate effectively with patients, community members, and staff members.
  • Proficient computer skills, including typing, internet navigation, and email use
  • OSHA training
  • BCLS biannually

General Duties include but are not limited to:

  • Collaborate with medical providers and staff to identify patients via referral from a target PCMH population who clinically demonstrate (i.e. high ER utilizer, uncontrolled hypertension) need of care coordination.
  • Assist the healthcare team and patient in developing patient-centered care plans tailored to individual needs and social support systems.
  • Obtain patient consent for care coordination and assess disease understanding, treatment compliance, depression risk, laboratory results, medications, and health-related goals.
  • Complete ER/Hospital follow-up calls in a timely manner, with the goal of scheduling a provider visit within 7 days of discharge.
  • Assess patient’s social determinants of health (SDOH) and provide appropriate information on community resources.
  • Other duties as assigned.
